This form contains questions to critical information that is beneficial to emergency service professionals and first responders in emergency situations. Persons that should submit are amongst those that special consideration must be made in order to successfully evacuate a residence should the need arise. CCDES, Police agencies, and Fire Departments utilize this information as a planning tool to determine the resources that may be required to provide assistance in the event of a major emergency or disaster. CCDES intend to update our current list that was created in 2009 and maintained over the past three years by an opt-in volunteer submission. Part of the updating process is deleting old or outdated information.
